Vision Program Overview
It’s important to get your vision checked on a regular basis, even if you aren’t experiencing any issues or changes in vision. So even if you’re one of the lucky ones who has perfect vision, don’t pass up the opportunity for a free eye exam. Believe it or not, your eyes provide a snapshot into your body’s overall health!
Get an Eye Exam + Glasses
HAAM’s Vision Program is partnered with CommUnityCare to offer a free basic eye exam and pair of glasses (if needed) to HAAM members once every two years.
Steps for Using the Vision Program:
Fill out the form below to submit your request
Receive an email from HAAM with your digital vision voucher and scheduling instructions
Schedule your appointment with the participating provider of your choice
Present your digital voucher at your appointment and get your exam!
Pick-up your glasses (if applicable) from the vision provider once they’re ready (the provider will contact you)
IMPORTANT: Participating vision providers are unable to offer progressive lenses, exams/prescriptions for contacts, exams for medical eye issues or urgent appointments.
